Medicare Patients Only: The ACCESS Model program is available exclusively for patients with Medicare insurance. Please confirm Medicare eligibility before submitting a referral.

The CMS ACCESS (Advancing Chronic Care, Equity, Sustainability, and Safety) Model is a federal value-based care initiative designed to improve outcomes for Medicare patients with kidney disease through coordinated, team-based nephrology care.

How to Refer a Patient

Referring a Medicare patient to our ACCESS Model program is simple. Follow the steps below — we'll handle the rest and keep you informed.

Confirm Patient Eligibility

Verify the patient has Medicare insurance. The ACCESS Model is exclusively for Medicare beneficiaries. Confirm the clinical indication warrants nephrology consultation (CKD, hypertension, transplant candidacy, dialysis, etc.).

Collect all required details before sending (see the checklist below). Having complete information ensures the fastest possible scheduling for your patient.

Please include all of the following in your referral to ensure prompt scheduling and the highest quality of care for your patient.

Patient Name

Full legal name as it appears on insurance

Date of Birth

Required for patient identification and scheduling

Medicare Insurance Information

Medicare ID / MBI number and any secondary insurance

Clinical Reason / Diagnosis

Reason for referral, relevant ICD-10 codes, and clinical indication

Current Medications

Complete medication list including dosages

Any Relevant Clinical History

Lab results (BMP, urinalysis, GFR trend), imaging, prior nephrology notes, or other pertinent clinical information

Patient Eligibility

The ACCESS Model program serves Medicare patients with kidney-related conditions.

Insurance Requirement

This program accepts Medicare only, including Medicare Advantage (coverage verification required).
