Referring a Medicare patient to our ACCESS Model program is simple. Follow the steps below — we'll handle the rest and keep you informed.
Verify the patient has Medicare insurance. The ACCESS Model is exclusively for Medicare beneficiaries. Confirm the clinical indication warrants nephrology consultation (CKD, hypertension, transplant candidacy, dialysis, etc.).
